Blue Coolant Sensor replaced, no change.
O2 sensor was replaced as well. No noticeable change, though fuel economy improved.
Fuel pressure regulator also faulty and replaced. Improved situation after periods of sitting (as it now holds fuel pressure properly).
Vacuum hoses replaced. No change (none was expected).
Vacuum elbow connecting to intake pre-heat valve was old and deteriorating. Replaced. No change.
Further investigated all aspects of intake pre-heat system. Found that thermostatic valve was installed upside-down. Removed from airbox top, then found that the thermostatic valve gasket was old and not sealing. Replaced gasket with another used piece, and reinstalled valve in correct orientation. Confirmed visually that the intake pre-heat system is now functional.

This resolved this issue. Cold starts are no longer an issue.
